Because of the professional time involved in processing requests for medical records in addition to the copying charges we are required to collect a fee of $50 fee prior to beginning the process. Once you have completed this portion of the process you may go back and complete your request so that we may begin processing it.

Please note - we accept VISA and MasterCard for online payments (not American Express or Discover).  As PayPal processes our payments you may also make a payment using your PayPal account although you do not need a PayPal account to use this service.
